Remote Patient Monitoring

Improve your patients' health by enabling them to collaborate with you at home.

 

You will also enable new billing codes, here are a few:

  • 99453 - One time code with reimbursement ~$21 for helping a patient setup a remote monitoring device, and teaching them how to use it.

  • 99454 - Once per month with reimbursement ~$69 for supplying the patient with remote monitoring device.

  • 99457 - Once per month with reimbursement ~$59 for spending 20+ minutes communicating interactively with patient or caregiver concerning remote care treatment management 

Step 1 - Enroll the patient in a care plan

Agree with the patients on what should be monitored at home and set up the Care Plan for the patient. We provide care plan templates for the most common chronic conditions, e.g. hypertension, diabetes and asthma, but you can also set up custom care plans for the patient

Step 2 - Patient monitors vitals through the app

After you have set up the care plan, the patient downloads the MyCarePlan app and it will guide the patient every day, with tasks for what they need to monitor and when. Data is either manually entered by the patient or the patient can use bluetooth integrated devices.

Step 3 - You review the data and import to EMR

The care team reviews the data on easy-to-use graphs that give you an instant overview of how the patient is doing at home - data is then imported into the EMR with the click of a button.

You can also set alerts to warn you when patients are falling out of bounds.

Step 4 - Bill once for setup and then monthly for device access and time spent communicating
